Tax Preparation List

This tax preparation checklist can help you organize your tax documents and provide you with a reference during the year on which documents to save in particular. While tax laws change from year to year, the basic information we will need to prepare your return each year is listed below:

Personal Information:

Social Security Numbers (including your spouse, children and any dependents)
Your child care provider's name, address and tax I.D. or Social Security Number

Employment / Income Information: Homeowner/Renter Information:

W-2 forms Residential address(es) for the tax year
Unemployment compensation (1099-G) Mortgage interest (Form 1098, provided to you by your lender)
State and local income tax refunds (1099-G) Second mortgage interest paid
Miscellaneous income (1099-MISC) Sale of your home or other real estate (Form 1099-S)
Pensions and annuities (1099-R) Real estate taxes paid (incl. school, county, city, etc.)
Social Security/RR1 benefits (RRB-1099) Rent paid during tax year
Partnership, S Corporation and trust income (Schedules K-1)
Alimony received
Jury duty pay
Gambling or lottery winnings (W-2G)
Prizes, awards, scholarships and fellowships (1099-MISC)
Prizes, awards, scholarships and fellowships (1099-MISC)

Expenses: Financial Assets/Liabilities:

Charitable contributions (cash and/or property) W-2 formsInterest income statements (1099-INT & 1099-OID)
Unreimbursed expenses related to your job or volunteer work Dividend income statements (1099-DIV)
Investment expenses Proceeds from broker transactions (1099-B)
Child care expenses Retirement plan distributions (1099-R)
Adoption expenses Student loan interest paid
Job-hunting expenses Auto loans and leases, if vehicle is used for business
Job-related educations expenses (account # and car value)
Medical/dental/vision expenses (co-pays, etc.)

Self-employment Information: Miscellaneous Information:

Business income (1099-MISC, and/or own business records) Federal, state and local estimated income tax paid during tax
Partnership SE income (Schedules K-1) year (vouchers, cancelled checks)
Business-related expenses (receipts, financial records) IRA, Keogh and other retirement plan contributions
Farm-related expenses (receipts, financial records) Any casualty or theft losses
Employment and business taxes paid during tax year Other expenditures that may be deductible
